Apply Online for Admission to the
Tennessee State Veterans Homes
Facility of Your Choice

This new feature of our web site allows you to begin the process of applying for admission to a Tennessee State Veterans Homes facility from the convenience of your home or office. We realize that many of your (or your loved one's) important documents are filed away and it may be impractical to bring all your records when you visit one of our facilities. Therefore, you can complete the admission form (VA Form 10-10EZR) at your convenience using the instructions described below and E-mail it directly to us for prompt consideration. Please note that applying online is not a requirement for admission and this feature is solely provided for the convenience of some families.

The Five Step Process Is Easy!

Please complete the five steps described below to download the application, complete form and E-mail it to the Tennessee State Veterans facility of your choice.

STEP ONE: Check Your Computer System's Capabilities

What You Need to Apply Online

  1. The latest version of the Adobe Reader program. This free software is available from Adobe Systems Incorporated by clicking the button below and following the instructions. If you have difficulty opening the application form or typing your information onto it, you may need to update your software to the latest version.



  2. The ability to download and save files on your computer.
  3. If you are at a library or other public computer, this may not be possible.

  4. The ability to send E-mail including attachments from your computer. If you are at a library or other public computer, this may not be possible.

STEP TWO: Download the Department of Veterans Affairs Form 10-10EZR.

Click on the link below to download the form. If Adobe Reader is installed, the form will open in Adobe Reader. Click File, then Save As or (Save Page As) to save a blank form on your computer. If Adobe Reader has not yet been installed you may receive a pop-up window that allows you to "Open" or "Save" the file. When prompted to "Open" or "Save" the file, select "Save".

STEP THREE: Complete the Form

The form, created in portable document format (PDF), is designed to be filled out on your computer. You may click on a blank space and type directly onto the form. Check boxes can be checked by clicking on them. If you need to change any information, you can click on the blank space and edit your work using the delete and/or backspace keys. Please read "Step Four" before saving your work.

STEP FOUR: Save the completed form to your hard-drive.

It is important to save the completed or partially completed form to your computer's hard drive using a different file name. After working on the form in Abobe Reader, click "File", then "Save As", then type your new file name. Please make note of the location of the folder (or directory) in which the file is saved since you will need to retrieve the file to continue working on it or to E-mail it to a facility.

STEP FIVE: E-mail the completed form to the Tennessee State Veterans Home of your choice.

The completed form can be printed and/or E-mailed to the facility of interest to you.

First, save the completed form to your computer's hard drive using the information in "Step Four". The form can then be E-mailed as an attachment to either of the E-mail addresses shown below.

To send a file as an attachment to an E-mail, first create an E-mail using an E-mail address from above (or by clicking on an E-mail address from above). In your E-mail program (e.g., Outlook Express), click "Insert", then "File", then navigate to and highlight the saved file you would like to send, and click "Insert".

Alternatively, you may locate the file you want to send in Windows Explorer, right-click on the file, select "Send To", then select "Mail Recipient". The E-mail window will appear and you can insert the E-mail address from above into the "TO" data field.

Adobe Reader also has a button on its task bar that allows you to send the file as an attachment to an E-mail. Save the file (as in Step Four) before using this feature to insure that you have a completed copy on your hard drive and that your information will be included in the attachment.

What's Next?

If you successfully completed the form and E-mailed it to one of our facilities, you should receive a call or E-mail correspondence within a day or two. If you do not receive confirmation that the form was received or have any questions or difficulty, please call us at the numbers above. The form may also be printed and delivered by hand, mail or fax.
